Saliva collection is non-invasive, simple and can be done in the privacy of the home or doctor's office
Specimen is mailed directly to Aeron in a prepaid mailer (sample is stable for at least 3 weeks at room temperature)
Testing is cost effective (much less than blood testing) and reimbursable by many insurance companies and Medicare

If you are interested in setting up an account to do salivary hormone testing on your patients, please click on the following link to display a  New Account Form (Adobe Acrobat® Reader required).  Print the form, fill it out and FAX it back to Aeron at (510) 729-0383. Or you may call Client Services toll free at (877-22-AERON) and Aeron will FAX or mail the form for your completion.

Methods of Specimen Payment
Payment must accompany submitted saliva and urine specimens and may fall into one of two categories:

1. Patient Pay: The patient may furnish payment - VISA®, MasterCard®, American Express®, Discover®, check or money order - with the sample. A  receipt will be sent to the patient that includes CPT and ICD-9 code(s) (provided by the practice) for possible insurance reimbursement.

2. Provider Pay: Aeron will invoice you monthly for the samples received.

Insurance

Aeron LIfeCycles is a contracted provider for Medicare, Blue Cross, Blue Shield and United Healthcare. The physician must provide an appropriate ICD-9 code(s). The patient will include a copy of the front and back of the insurance card with the sample(s). The physician should not collect payment for lab services from these patients. Any co-pays and deductables are the responsibility of the patient and certain HMO plans may still require prior authorization as with other lab services. Payment is required for all other insurance carriers but the receipt Aeron provides may be used to request reimbursement from the patient's insurance company.
